CDCS / WAIVERED SERVICES

CDCS SUPPORT PLANNING

This is a waiver service for those who are either on a waiver or have been accepted to join one. Our planners are DHS-certified and follow all of the requirements for assisting people in Minnesota. Our programs are tailored to a person's specific needs and usually include the following:

  • Develop a person centered plan

  • Find services and resources.

  • Make plans for operation and support orders and distribution.

  • Monitoring and problem solving are also part of the plan.

  • Speaking with your social worker is the best way to find out whether you qualify. If you do not have a social worker, please contact us directly.

Social Security Advocacy

In collaboration with Disability Benefits Solutions, we offer representation services for people who would qualify for Social Security’s SSDI and SSI disability benefits. We will work with you and help you understand all the options available under the Social Security Administration disability laws and regulations at every stage of the process. We will obtain supporting evidence to build the strongest possible case for you. If a denial occurs, we will file timely appeals and represent you before the Social Security Administrative Law Judges (ALJs) at any hearing office in the nation.

ELIGIBILITY:

  • Phyical or mental health condition (s) that hinders your ability to engage in gainful employment and earn at least $1220 per month

  • Your condition meets or equals a Listing. If your mental and/or physical condition meets or equals specific medial criteria for that diagnosis/illness/condition, you will be eligible at this step. If not, the analysis will go to the next steps.

  • Does your impairement prevent you from performing your previous hobs? If yes, you will be eligible at this step. If you, the analysis will go to the next step.

  • Does your impairment prevent you fom performing any other type of work? If yes, you will be found to be eligible at this step. If not, you will be denied.
